Vantis RapidCare™
Vantis RapidCare™ — Acute Care & ED Diversion
Rapid clinical intervention for high-risk members experiencing changes in condition, delivered through telehealth and home-based care when clinically appropriate.
Care When Members Need It Most
Changes in condition don't always happen during a scheduled visit. Vantis Health provides rapid clinical access designed to identify emerging health concerns early, intervene when clinically appropriate, and help prevent avoidable emergency department visits and hospitalizations.
Through telehealth, clinical triage, and home-based care, our clinical teams provide timely assessment and intervention while maintaining coordination with the member's existing providers and health plan.
Rapid Clinical Access
Depending on clinical need and program scope, services may include:
Same-day telehealth assessment
Clinical triage and escalation
Urgent home-based provider visits when appropriate
Evaluation of acute changes in condition
Medication review and reconciliation
Chronic disease exacerbation support
Post-ED and post-hospital follow-up
Coordination with primary care and specialists
Emergency department and EMS escalation when clinically indicated
Closed-loop documentation and health plan reporting
Supporting Appropriate Site of Care
For medically complex and high-risk populations, timely intervention can make the difference between managing a condition safely in the community and an avoidable emergency department visit.
Vantis Health's Rapid Response model is designed to help members receive care at the right time and in the right setting, while preserving appropriate escalation to emergency services when higher-acuity care is required.
Built for Value-Based Care
Rapid Response is integrated into the broader Vantis Health value-based care model. Interventions can be tracked alongside utilization and clinical outcomes, including:
Emergency department utilization
Hospital admissions
30-day readmissions
Post-discharge follow-up
Member engagement
Changes in condition
Care escalation and disposition
Potentially avoidable utilization
This provides health plan and provider partners with visibility into both the clinical intervention and its impact on member outcomes and total cost of care.
From Scheduled Care to Rapid Intervention
Home-Based Care + Telehealth + Care Coordination + Rapid Response
One connected care model designed to support members across routine, transitional, and emerging acute needs.
